Under the rule of the military government, Myanmar's western region, rich in resources, has been coveted by the army for many years. Children there are often forcibly recruited by the army, and massacres of civilians have become a form of entertainment for some soldiers... A volunteer team from a church trying to enter the area to provide humanitarian aid to refugees finds the reclusive Rambo (played by Sylvester Stallone) in Thailand, hoping he can help them navigate the waterways. Rambo believes the situation is unlikely to change and refuses to guide them but is eventually moved by the sincerity of volunteer Sarah (played by Julie Benz) and agrees to take them there. Unfortunately, the volunteer team is attacked by the army in a village, and all survivors are captured. The church learns about the volunteers' perilous situation and organizes mercenaries for rescue, once again entrusting Rambo to lead the way. This time, Rambo's attitude changes; he leads the mercenaries into a Myanmar army camp and transforms once more into a passionate warrior.
