One night, a group of medical interns at a nearby bar play pranks on Kenneth, a hospital janitor with a voyeuristic tendency, mocking him as 'mad dog'. Forced to drink excessively, Kenneth suffers an epileptic seizure and falls into a coma, becoming a vegetable, with plans to terminate his treatment and carry out euthanasia. Overwhelmed by guilt, intern Katherine, played by Arielle Kebbel, violates protocol by injecting Kenneth with an experimental mixed drug in an attempt to wake him up early, but unexpectedly, the drug causes Kenneth's soul to leave his body and take over others to begin a cruel revenge spree.
