The story is set in a post-apocalyptic world where most of humanity has already died, and the world is ruled by mutated cold-blooded animals. The remaining humans are forced to live in underground bunkers, constantly living in fear. Joey (played by Dylan O'Brien) became separated from his girlfriend Amy (played by Jessica Henwick) during a disaster seven years ago, and now they reside in two different bunkers, communicating through radio. One day, deeply missing Amy, Joey decides to leave the safety of the bunker and venture back to the surface, embarking on a journey on foot to reach Amy's camp. Along the way, he encounters countless dangers, as well as a mentor and friend, Clyde (played by Michael Rooker) and Minho (played by Ariana Greenblatt).
