After the hurricane, Orleans was paralyzed. In the local prison, all the guards and inmates had been evacuated, except for one prisoner who was forgotten there. Officer Terence McDonagh (played by Nicolas Cage) discovered him while patrolling the cells, and Terence stepped in to help. This brave act earned him a promotion to lieutenant, but his spine was severely damaged, leaving him reliant on medication for pain relief. The worsening pain rendered various medications ineffective, and Terence suffered immensely during this process, turning to drugs for help, with both his physical and financial condition nearing collapse, and his temper getting worse. A heroic policeman gradually descends into decline. Soon, a massacre occurs in the city, with major suspicion falling on drug lord Dafeet.
